1- Parent\/Guardian Commitment<\/span><\/h3>
  1. Medical Fitness Health Declaration<\/strong>I, as the parent\/guardian, declare that the player is medically and physically fit and does not suffer from any chronic diseases or health conditions that would prevent them from practicing sports activities normally.<\/li>
  2. Understanding the Nature of the Game and Its Risks<\/strong>I acknowledge that I fully understand and agree that football is a sport that naturally involves physical contact, and I understand that accidental injuries may occur as a result of the normal practice of the game during training sessions or matches.<\/li>
  3. Consent to Photography and Media Publication Rights<\/strong>I consent to the academy photographing and filming the player (photographs and video recordings) during training sessions and all related activities and events. I grant the academy full permission to use and publish these materials on its official social media pages for documentation and marketing purposes.<\/li><\/ol>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

    \n\t\t\t\t\t\t\t\t\t

    2- Suspend Subscription<\/span><\/h3>\nSubscription Cancellation Instructions<\/strong>\n\nTo ensure smooth workflow and maintain the continuity of the training process efficiently, we would like to inform you of the following instructions regarding temporary subscription suspension requests at the academy:\n
      \n \t
    1. The suspension request can be made only once during the subscription period, and no more than one suspension request is accepted for any active subscription.<\/li>\n \t
    2. The maximum suspension period is one week only, and this period cannot be extended under any circumstances.<\/li>\n \t
    3. Personal absences are not compensated, and the subscriber acknowledges that if they are absent for any reason outside the academy-approved suspension, no make-up sessions or rescheduling will be provided.<\/li>\n \t
    4. In the case of a medical excuse documented with an official report, the subscription can be suspended for the period specified in the report, provided that the report is submitted to the administration as soon as possible for review and approval.<\/li>\n \t
    5. The suspension request should be submitted in advance through the link below.<\/li>\n \t
    6. The academy reserves the right to reject requests that do not comply with the above conditions or that affect the training program's workflow.<\/li>\n<\/ol>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

      3- Behavior and Discipline<\/span><\/h3>
      • The player agrees to adhere to the behavioral rules and discipline instructions provided by the coaches.<\/li>
      • Unacceptable behavior, such as using offensive language, will result in disciplinary action.<\/li>
      • Depending on the severity of the violation, athletes may receive up to three warnings. The player agrees that if they exceed three warnings, they may face suspension from the academy, ranging from a one-week suspension to an indefinite suspension.<\/li>
      • Hitting \/ Fighting: Hitting and fighting are considered major violations. The player agrees not to hit or engage in any altercations. The player acknowledges that violating this agreement will result in suspension or termination of their subscription without refund.<\/li>
      • The academy is not responsible for injuries resulting from failure to follow instructions.<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

        4- Uniform<\/span><\/h3>
        • The player must wear their official uniform during training and is required to maintain it properly.<\/li>
        • The player agrees that the academy is not responsible for damaged or lost uniforms.<\/li>
        • If the player loses their official uniform, they must purchase a new one.<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

          5- Refunds<\/span><\/h3>

          Refunds can be issued in the following cases only:<\/p>

          • A medical report indicating that the athlete can no longer participate in any sports activities due to a medical diagnosis stating that the athlete is unable to engage in any sports activities.<\/li>
          • Fees can be refunded within 7 days of registration if training has not started. Please note that once the program has begun, fees are non-refundable.<\/li>
          • If the program is canceled by the academy, the full payment will be refunded.<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t
